Evidence Approval State. The governed review state of evidence for a specific use: approved, pending, rejected, revoked, or otherwise not yet eligible to ground a conclusion.
Approval state is a Truth Plane fact about evidence, not a side effect of indexing, retrieval, or model context. A document can be fetched, parsed, embedded, and ranked, yet still be pending or rejected for the use in front of the system.
Evidence approval state feeds evidence eligibility, source health, and confidence gates. If the state is pending, rejected, revoked, or missing, the honest system blocks grounding, routes review, or asks for approved evidence instead of laundering the source into an answer.
