Known Gap. An explicitly recorded absence in evidence, coverage, implementation, review, or readiness. A known gap is not a defect to hide; it is a governed state that lets a release, review, or decision remain honest about what has not yet been proven, finished, or approved. When the gap blocks a requested state, a conflict explanation should say which facts or constraints prevent progress.
When the gap is missing required information for a workflow, a data-request blocker gives the gap an owner, dependency, and resolution condition.
